The First Line of Defense
Topical treatments are treatments that are applied directly to the skin. When treating psoriasis, topical treatments are usually the first line of defense. These treatments may improve the skin’s condition. They are available both as OTC (over-the-counter) or by prescription. There are several different types of topical treatments that your doctor may recommend. Your doctor may also try phototherapy or systemic treatments in combination with topical treatments.
Corticosteroids: Corticosteroids reduce the inflammation of the skin. Corticosteroids come in an ointment or cream form and range in strength. You can purchase a low-strength corticosteroid without a prescription at your local drug store; however, if you require something stronger, you must consult your doctor for a prescription. Of note, long term or overuse of this product can cause a thinning of the skin.
Calcipotriene: Calicipotriene is a synthetic form of vitamin D3. Calcipotrine comes in the form of an ointment. It is applied to the skin twice daily; which slows the speed of turnover of skin cells. This product may cause some irritation.
Retinoids: Retinoids are a synthetic form of vitamin A. This topical treatment is available in a gel or cream. Retinoids may irritate the skin and might be used in combination with corticosteroids to achieve the desired effect. Vitamin A can minimize the thickness and scaliness of skin.
Coal Tar: Coal tar can come in either gel or cream form. You can apply this treatment directly to the skin, it can be added to a bath, or used in a shampoo. Coal tar has the reputation of having a strong odor and being extremely messy and it may stain skin and clothing. There are new brands that come in a clear form and have a low odor. This topical treatment is available without a prescription.
Anthralin: Anthralin reduces the increase in cells and inflammation. Typically, a doctor will prescribe a 15-30 minute application. Once the time is up, you must wash off all of the product. Like coal tar, this product is messy and may stain clothing and skin, as well as any surface area where you are applying the product.
Salicylic Acid: Salicylic acid is a peeling agent commonly used to treat acne. This product is used in the treatment of psoriasis to control the scaling of skin that occurs.
